Cosmos Midwifery offers pregnancy, home birth, and postpartum care; fertility counseling and at-home inseminations; annual and problem reproductive visits; and gender-affirming hormone therapy.

 

Pregnancy, Homebirth, and Postpartum Care

Prenatal care starts with an introductory visit in the first trimester, followed up by a visit once every 4 weeks until 28 weeks, then every 2 weeks until 36 weeks, and then every week until the baby arrives. These visits take place either in the sunny Cosmos Midwifery office or your home, and run usually about an hour long. Prenatal visits include any pertinent lab work, vital signs, listening to your baby’s heartbeat, measuring their growth, and checking their position. Most of the appointment, however, is taken up with talking, as Chloë and Jenna listen and guide or support you in whatever ways you need while coping with all of pregnancy’s ups & downs.

When you go into labor, either Chloë or Jenna will be in direct contact with you by phone and text, and when you enter active labor the on-call midwife will come with a trained birth assistant to your home to offer labor support and respectful monitoring of you and baby throughout. She will stay at your home usually around 3-4 hours after you give birth to make sure all is well. She will complete a newborn exam, tidy up your home, and tuck you into bed.

In-depth postpartum care is a central principle of Cosmos Midwifery’s practice. You can expect to have a home visit from the midwife 1-2 days after you give birth, a lactation consultant in the first week, and a postpartum doula at 2-3 weeks postpartum. You will see Chloë or Jenna again virtually at 2 weeks, and in the office at 6 and 12 weeks postpartum. Throughout all of that time, the midwives stay in touch 24/7 by phone and email.

Fertility Consultations and Intrauterine Inseminations (IUI)

Whether you plan to conceive with fresh or frozen sperm from a partner, a donor, or via intercourse, this consult is key to a positive and successful conception process. Together with the midwives, you will work through your health history and current lifestyle – your nutrition, stress levels, health history, current menstrual cycle / sperm health, and plans for the future. The goal of this 2-3 hour intake visit is to establish a personalized support system that maximizes your comfort and fertility. After this visit, you have 24/7 email access to the midwives for any extra support, chart reviews, lab work, and help timing intercourse or inseminations.

If you’re using fresh or frozen sperm and hoping to use Intrauterine Insemination (IUI) to conceive, either Chloë, Jenna, or one of their back up midwives will either come to your house to do the IUIs on your bed or couch at the time of your ovulation, or you can come in to the office to have it done here. Instead of stimulating your ovaries with medications and triggering ovulation with an injection, the midwives monitor your body’s individual signs of fertility and ovulation, timing IUIs around your natural cycle.

Well-Body Care and Gender-Affirming Hormone Therapy

Chloë and Jenna offer lifelong reproductive healthcare for all people in all kinds of bodies. During an hour-long visit in the office, the midwife will review your history and help you optimize your health in the future. These visits may include:

 
  • Reproductive organ cancer screening like the Pap Test (Pap smear, Papanicolaou)

  • Sexually Transmitted Infection testing

  • Assessment of and treatment for vaginal infections like yeast or Bacterial Vaginosis

  • Sexual pleasure and comfort

  • Safety in relationships

  • Becoming pregnant or preventing pregnancy

  • Holistic ways of addressing gynecological issues such as heavy, crampy periods, irregular spotting and bleeding, symptoms of PMS and hormone imbalances

  • Gender-affirming hormone therapy
